THE EFFECT OF MODE DELIVERY IN INITIATION OF LACTATION

ABSTRACT
Background: Sixteen percent of neonatal deaths could be saved if all infants were breastfed from the first day and 22% if breastfeeding started within the first hour.
Objective: To discover the effect of mode delivery in initiation of lactation.
Methods: Prospective cohort study. The study was conducted by taking all cases of vaginal delivery and cesarean section in Sardjito, Banjarnegara, Wates, Wonosari, and Magelang hospitals from January to May 2014 that meet the criterias. Maternal datas were collected from medical records, research datas obtained from observing ejection of milk within 24 hours post delivery by medical or paramedical personnel who have been trained. The statistical test used is the Chi-square.
Result: The study subjects who eligible for inclusion criterias were 162 people. Based on the characteristics of the mode of delivery, initiation of lactation occured on the first day of post vaginal delivery as many as 73 people (90.1%), whereas 34 people (42%) in the cesarean delivery. There was no significant relationship between age, education level, occupation, and parity with the initiation of the first day of postnatal lactation. Statistically, BMI <25 kg/m2 had a significant association with the initiation of the first day of postnatal lactation, but not clinically significant. There is a significant association between mode of delivery with the first day of postnatal lactation initiation (OR=20,17; 95%CI 7,47-54,43; p=0,000).
Conclusion: The proportion of lactation initiation in the first day of postvaginal delivery was greater than cesarean delivery.
